The Basics

What is Eyelid Surgery?

As one ages, the signs of fatigue around the eyelids become increasingly noticeable. The eyelid skin wrinkles, the eyelid muscle enlarges, and the fat underneath begins to bulge. Each patient ages differently and to combat natural aging, Dr. Paul tailors upper and lower blepharoplasty procedures for patients seeking eye rejuvenation. During your consultation, the position of your eyebrow will also be discussed as the descent of the brows often crowds the upper lids reducing the distance from the eyebrow to the eyelashes.

The goal of eyelid surgery is to restore a youthful and energetic look to the eyes that matches the patient’s age and exuberance.

Upper Eyelid Surgery

The main component of an upper blepharoplasty is the removal of excess skin, a condition named dermatochalasis. This will decrease the “hooding” of the eye and present more eyelid platform, which women often appreciate as it facilitates the application of eyelid makeup.

The excess skin is removed through an incision hidden in the natural eyelid crease, rendering the resultant visible scarring almost virtually invisible. For some patients, a skin excision alone will suffice. For other patients, a thin strip of the squint muscle is removed to lessen the eyelid bulk. For some patients, prolapsing fat is excised as well to improve contours. Care is taken to not over-reduce soft tissue or hollow the eyes.

During the evaluation process, careful attention is given to the eyebrow position, as brow descent may give the appearance of excess skin. An endoscopic browlift may then be indicated in addition to, or instead of, removing eyelid skin.

Lower Eyelid Surgery

Lower blepharoplasty is a more complex procedure. In addition to prolapsed fat, stretched-out eyelid muscle, and skin may compromise the appearance of the the upper and lower eyelids together. Furthermore, cheek descent can worsen this appearance by creating a groove below the eyelid, often termed the “tear trough”. Swelling in the cheek/eyelid transition zone, termed “festoons”, can further complicate treatment.

These features may require a combination of approaches: fat may need to be removed or repositioned through an incision hidden on the inner surface of the eyelid (transconjunctival), or through an eyelid incision just below the lashes (subcilliary) if removing skin and tightening muscle is also planned. If skin creping or festoons are present, laser resurfacing may be performed to smooth and tighten the lower lid skin.

Eyelid Surgery Timeline

  • CONSULTATION (FIRST VISIT)

    Dr. Paul will look at your eyes and determine if you require an Upper Eyelid Surgery, Lower Eyelid Surgery, or both. The two of you will discuss your options and a surgical plan will be made. You will then schedule your surgical date with our Patient Coordinator.

    1st Visit
  • PREOPERATIVE VISIT (PRIOR TO SURGERY)

    You will meet again with Dr. Paul and/or the nursing team to prepare you for surgery. We will review instructions as well as determine if you need a medical clearance.

    2nd Visit
  • DAY OF SURGERY

    Dr. Paul will make discreet incisions along the natural creases in your eyelids and remove any excess skin or fat. Once done, he will suture your eyelids to a natural resting state and you will need to wear bandages over your eyes. For an upper lid blepharoplasty, you may return home after the procedure. For a lower lid blepharoplasty, you will be met by a private nurse and will be asked to stay in a neighboring hotel overnight for direct observed care.

    3rd Visit
  • DAYS FOLLOWING SURGERY

    You will experience bruising, swelling, and discomfort, but can return to work if you feel ready, but it’s better to take about a week to let your body heal. Refrain from any strenuous activities or lifting during the first two weeks. You will need to return for a follow-up with Dr. Paul so that he can observe your initial recovery

    (DAYS 1-7)
  • Weeks Following Surgery

    Swelling and bruising around your eyes will have decreased significantly or diminish during this period. You should be seeing the results of your blepharoplasty at this point, and your eyes will look more open and refreshed. Makeup can be applied at this point and you may also return to light to moderate exercise around 3-4 weeks after the procedure following our guidance.

    (WEEKS 2-4)
  • FINAL RESULTS

    Upper eyelids heal very rapidly. Lower eyelids can heal rapidly, but may take longer to heal depending on approach. You may need to return to Dr. Paul a few more times over the next few months so that he can monitor your progress. Once fully healed, you will be able to enjoy the results of your Blepharoplasty for the rest of your life.

What to Expect on the Day of Eyelid Surgery

When you come in for your Blepharoplasty, we want all of our patients to feel as comfortable and confident as possible while undergoing eyelid surgery. You usually have just local anesthesia around your eyes and mild sedation during the procedure.

  • Upper Eyelid Surgery

To address the upper lids, Dr. Paul cuts along the natural lines of your eyelids and then removes any excess tissue. He uses extremely small stitches to close the incisions. The stitches stay in place for one week. This addresses eyes that appear hooded or droopy due to excess sagging skin and fat. Excess sagging eyelid skin can make an individual appear tired, sad, or older. Upper eyelid surgery rejuvenates the eye area, helping patients look younger and well-rested.

During surgery for the upper eyelid, the incision is typically hidden within the natural fold of the upper eyelid, extending slightly beyond the lash line at the outer corner into the laugh lines or existing creases. Since this incision follows the natural contour of the upper eyelid, it is usually nearly undetectable.

  • Lower Eyelid Surgery

Dr. Paul uses one of two procedures to address the lower eyelids. He may make an incision on your lower eyelid to remove fat. The advantage of this procedure is that the cut will be invisible. Alternatively, he may cut the eyelash area, and this incision fades in a short time.

Lower eyelid surgery addresses puffy under-eye bags, loose skin, and hanging tissue in the lower eyelid area. Many patients come to us seeking this procedure because they’re simply tired of looking exhausted. Similar to upper eyelid surgery, incisions made for lower eyelid surgery are hidden in natural crease lines just below the lower lashes. If the patient is only having fat removed and not skin, the incision can be placed inside the lower eyelid, eliminating the need for external incisions. Lower eyelid surgery incisions may be combined with laser treatment to tighten and smooth the lower eyelid skin.

If both your upper and lower lids are done at the same time, the entire eyelid surgery procedure usually takes two hours.

Eyelid Surgery Recovery

After surgery, you will be able to return home to rest in comfort, however, you should have a friend or family member available to take you home. You should take it easy for the first few days and have a competent adult around to help you during your initial recovery. After surgery, it is common for your eyelids to be puffy, as your incisions will emerge red, and the swelling and bruising may resemble that of a black eye.

Most people can get back to their regularly scheduled activities in about 7-14 days after Eyelid Surgery, but you should be prepared to take time off to let your eyelids heal correctly. Within this healing time, we advise that you must protect your eyes from sunlight and other elements. Some choose to wear dark sunglasses that cover your eyes from the sides and a hat. We always ask that you follow your post-surgery instructions from Dr. Paul.

Once you are fully healed, you’ll look younger, your eyes will look brighter, and you’ll hopefully have an elevated sense of confidence.
