What is a Hair Transplant?

Hair restoration focuses on re-establishing a natural-looking hairline and improving overall hair density. Dr. Paul recognizes that each gender has a separate pattern of hair loss which is influenced by life events and genetics. Dr. Paul performs personalized hair restoration that is easy to maintain and compliments the face in a natural, age-appropriate way.

The treatment of hair loss can be approached from many directions. Often combination treatment is more powerful than any single treatment. Dr. Paul is versed in all aspects of hair restoration. At your consultation, Dr. Paul will review your relevant medical history to see if there is an underlying cause other than familial hair loss that can be reversed. Dr. Paul will provide recommendations on non-surgical options for care such as vitamins, topical treatments, oral medications, and light therapy.

Types of Hair Transplants

For patients with active, focal, or established hair loss, a hair transplant is a definitive technique. Hair transplant techniques have improved remarkably over the past decade. Embracing these advancements, Dr. Paul provides patients with two different surgical restoration options to meet individual hair expectations and long-term goals.

The most common options, Follicular Unit Extraction (FUE) and Follicular Unit Transfer (FUT) differ on how the donor’s hair is gathered but provide equally aesthetic beautiful outcomes. Each technique has specific advantages and Dr. Paul will help you decide the best option during your consultation if you are a candidate for hair restoration surgery.

  • FUE – During a FUE Transplant, hair follicles are transferred individually, from the back to the scalp, where the hair grows throughout life and is less sensitive to hormones. This innovative procedure has been helping patients regain their own healthy set of hair through rapid and precise technology. This is the ideal option for men and women who are looking for a permanent solution to address their hair loss concerns. In an FUE procedure, there are no scar lines, no scalpels and no sutures.
  • FUT – With FUT, a “donor strip” is harvested from the back of the scalp and individual follicles of hair are dissected after the strip is removed. The donor site is sewn closed. The hair above the donor is left long to cover the suture line. The strip of hair bearing tissue is dissected into the individual follicles.  The hair follicles are then transplanted to the area where the hair is thinning.
  • ARTAS iX Robotic – The ARTAS® robotic hair transplant system intelligently analyzes and selects the appropriate grafts from a donor area without the risk of linear scarring. This is a form of FUE and only is applicable in a small minority of patients due to technological constraints.

Hair Transplant Timeline

  • CONSULTATION (PRIOR TO SURGERY)

    When you come in for your Hair Transplant consultation, Dr. Paul will examine your scalp and discuss the pros and cons of the various treatments we offer.  Great care will be taken to consider your current pattern of hair loss, what the future may hold, and what your goals are.  After determining the best technique for you, he will create your treatment plan, take photographs, and you will meet with our Patient Coordinator to select a procedure date.

    1st Visit
  • PREOPERATIVE VISIT

    The goal of the pre-operative visit is to get you ready for your upcoming hair transplant. You will be provided a packet with complete instructions so you can optimize the experience and result. All medicines will be sent in for you in advance. This visit can be done in person or virtually.

    2nd Visit
  • DAY OF SURGERY

    On the morning of surgery, you will meet with Dr. Paul and confirm your plan. The area into which hair will be placed will be marked. A hair transplant has 3 parts – harvest of hair from the donor, creating of sites (pockets to accept the hair) in the recipient, and placement of the hair into the sites.  Some patients will be given oral sedation during this procedure and some will be under deeper anesthesia.

    3rd Visit
  • DAYS FOLLOWING SURGERY

    The headwrap is removed the morning following your procedure at home. You will be instructed to put bacitracin on your donor site and to spray your recipient site with saline. Most may wear a baseball cap the next day. Most can work from home the next day.

    (DAYS 1)
  • DAYS FOLLOWING SURGERY

    You will need to spray your scalp regularly with a special solution to speed up the healing of the follicles, protect against infection to the incisions, and soften scabbing. Be careful when touching your hair to prevent any bleeding or irritation. Avoid sweating or strenuous activity.

    On day 2 after the procedure, many can do a modified gentle hair wash. We will guide you with this depending on the details of your procedure.

    (DAYS 2-5)
  • DAYS FOLLOWING SURGERY

    You will stop spraying the head and stop bacitracin. Most patients have reduced swelling at this point.  Most can return to work. Continue to avoid sweating or strenuous activity.

    Most patients will follow up around day 10 after the procedure to assess progress and to help you return to regular life.

    (DAYS 5-10)
  • FIRST WEEKS FOLLOWING SURGERY (WEEKS 2-4)

    Your scalp should be healed enough to gently wash it since the soreness and swelling will have mostly disappeared.

    At 3 weeks, most patients may return to full exercise.

    (WEEKS 2-4)
  • MONTHS FOLLOWING SURGERY (MONTHS 1-6)

    Your follicles will go dormant and your new hair will fall out – don’t worry, this means the treatment is working. The hair has to cycle.  Most patients see little growth at 4 months after the procedure as the new hair follicle is resting and getting ready to grow new hair.

  • MONTHS FOLLOWING SURGERY

    At 6-8 months after surgery, most people are seeing signs of success from their hair transplant. You will visit with Dr. Paul if available to assess your progress.

    (MONTHS 6-12)
  • FINAL RESULTS

    The majority of your follicles should be growing hair and we hope you are enjoying your result. While a hair transplant can last you for years to come, it does not combat the natural aging process of balding, but it will extend your timeline.  You should continue to visit the clinic to ensure we are taking steps to protect your original hair from receding further.

What’s The First Step?

If you feel like you could benefit from a  New York City Hair Transplant, the first step is to contact our office and schedule your consultation. During this appointment, you will be able to meet with your plastic surgeon and discuss every question, concern, and goal you have with the hair transplant surgery.

Dr. Paul will ask you about your medical history and examine your hair to determine the best methods when creating your surgical plan. When discussing your medical history, we encourage you to be very open and honest as all the information helps make the best decisions and prevent unnecessary risks.

If you agree with the plan created for you, you will then speak with our Patient Coordinator to schedule the date and time of your surgery. You will also be given some instructions to follow before coming in so that you will have the most optimal surgery and recovery.

What to Expect on the Day of Hair Transplant Surgery

Your experience will be a little different depending on which technique you and Dr. Paul settle on, but the steps are very similar.

At the start, you will meet with Dr. Paul and re-discuss the goals of the day. The recipient site will be marked. You will either have oral sedation or deeper anesthesia, and this decision will be determined during your initial consult prior to the date of the procedure.

For FUE (folliculuar unit extraction), the “donor area” will be trimmed short to about 1 – 2 mm in length so the grafts can be more easily accessed and removed. You will be in the prone position (flat on your stomach) on the operating table and Dr. Paul and his team will begin performing your hair transplant. The procedure is performed with injections of numbing local anesthesia so you do not feel pain.  A small microdrill will release the individual hair follicle from the scalp and a micro-forcep will lift the hair once it has been released. The hair follicle (also called graft) will be preserved in a saline solution. Each graft will usually consist of 1 – 4 hairs. This is a very technical and meticulous process, and Dr. Paul and his team have truly mastered it.

Once the extraction is complete, Dr. Paul will create sites in your scalp. Each site is a pocket into which a graft is placed. This step sets the artistry of the result because the distribution and even angulation of the site will matter in your result. Dr. Paul is very careful to use your existing original hairs to guide the site making so your transplants and original hairs blend seamlessly.

The final step of your hair transplant is placement of the hairs into sites. Usually, two team members will place simultaneously to maximize efficiency and comfort. Once the hairs are placed, a wrap is placed over your head. Instructions are reviewed for after care and you are able to return home.
