Introduction

MOH’s Reconstruction

Dr. Paul prides himself on delivering natural, long-lasting results with minimal down time.

When a patient has skin cancer, a dermatolgist will remove the cancer with a technique called MOHs. This will leave a hole that needs to be repaired, which is often performed by a plastic surgeon. In the face, reconstructive excellence is especially important. Dr. Ben Paul is a uniquely trained, double-board-certified Facial Plastic Surgeon with extensive experience in MOH’s Reconstruction surgery after skin cancer removal in New York City.

Dr. Benjamin Paul performs the reconstruction after MOHs has been performed by one of Dr. Paul’s dermatology colleagues to remove skin cancer of the head or neck. If you need a referral for MOHs surgeon to perform your excision, we can help recommend you to an excellent MOHs surgeon and then help you with repair.

The Basics

What is MOH’s Reconstruction Surgery?

Once all of the cancerous tissue has been removed by the dermatologist and the “margins” have been “cleared” then you are cancer free and eligible for repair. A gauze bandage will be placed on your wound and you will be sent to our office.

While there are multiple options for healing depending on the location, size, and depth of the defect, the most aesthetically appealing option for those wounds that are deeper or too large to heal on their own is MOH’s Reconstruction Surgery. Dr. Ben Paul of New York City offers this type of restorative surgery to repair the target area.

Depending on the size, shape, and placement of the defect, Dr. Paul performs a variety of reconstruction methods, such as flaps or grafts, to cover and repair the area.  The exact method and options will be tailored to your need and discussed with you prior to closure.

MOHS Reconstruction Timeline

  • CONSULTATION

There are times when skin cancer is identified and you can meet with Dr. Paul before your Mohs excision and reconstruction day.  There are other times that Dr. Paul will meet you for the first time because the dermatologist wishes for Dr. Paul to do the closure and did not expect such a large defect as the skin cancer was deeper or larger than had been anticipated. Dr. Paul will discuss your reconstructive options and plan your closure.

    PRIOR TO SURGERY
  • DAY OF SURGERY

Dr. Paul will repair you after your MOH’s surgery to achieve the best aesthetic result, and then bandage you up. After your surgery, you will be able to return home to rest in comfort, but there will be some moderate pain, bruising, and swelling. Depending on the extent of your skin trauma, post-Mohs reconstruction may be performed in multiple stages.

  • DAYS FOLLOWING SURGERY

Many will be able to return to work the day following surgery, provided that your job does not entail vigorous physical activity. You should plan on resting and icing in the first day or two after the procedure. Swelling peaks 2-3 days after repair.

Please continue to avoid smoking and any vigorous physical activity.

    DAYS 1-3
  • WEEKS FOLLOWING SURGERY

If Dr. Ben Paul uses sutures during your MOH’s reconstruction procedure, they are typically removed within 6 to 10 days of the procedure. Suture removal may vary depending on the site and tension placed on the closure. Most bruising should resolve by two weeks.

    WEEKS 1-2
  • MONTHS FOLLOWING SURGERY

Most of your wounds are fully healed, however, there may be some residual redness and swelling. It is very important to avoid excess sun as this can pigment the closure. Any scarring should continue to improve over the course of a year. We may guide you back to your dermatologist for skin recommendations and possible laser to help with healing. Then, you can discuss scar treatment options with your doctor to decrease redness and improve texture, both of which are completely normal, by the way.

    WEEKS 4-6
  • FINAL RESULTS

Once you are fully healed, your face should return to its original state. You will need to stay vigilant any time you are out in the sun by wearing the proper clothing to protect you and wearing a strong SPF.

Benjamin Paul, MD

What’s The First Step?

The first step in getting MOH’s reconstruction surgery with Dr. Ben Paul in New York City is to schedule a consultation at our clinic. On the day of your appointment, Dr. Paul will evaluate your wounds and scars from the skin cancer removal surgery. He will also evaluate a realistic expectation for the restoration of your facial anatomy.

What to Expect on the Day of MOH’s Reconstruction Surgery

On the day of your procedure, you will require anesthesia to be administered for your comfort and pain management. The procedure required for MOH’s Reconstruction Surgery depends entirely on the area, depth, shape, and placement of the facial defects post-skin cancer removal.

Some options for reconstruction include:

  • Primary closure, in which the skin of the Mohs surgical wound is pulled together and sutured
  • Grafting, in which skin from another area of the body is used to cover up the wound created by the Mohs surgery
  • Flap reconstruction, where nearby skin is moved to conceal the wound made after the Mohs technique

A proper MOH’s reconstruction procedure begins with creating an enviroment of comfort. We will give you numbing medicine so you feel no discomfort during the procedure. After this, we are focused on cleaning and sterility. Once the treatment site is prepared, Dr. Paul will use subspecialized techniques to repair your defect to achieve the best aesthetic result. Sutures are placed and a bandage is often placed at the conclusion of the repair.

Finally, the patient will be discharged with a detailed post-operative and at-home wound care plan. We highly encourage you to follow the instructions as inadequate healing can cause deformity, scarring, and other serious side effects.

MOH’s Reconstruction Recovery

Post-operative care for MOH’s Reconstruction Surgery is an important part of the recovery process. After your surgery, it is extremely important to follow the guidelines your doctor has suggested to ensure a positive outcome and prevent complications.

Post-surgery, you will be monitored in a recovery room until Dr. Ben Paul decides that you are safe to be discharged. You will need a family member or friend available to drive you home in case you are still feeling some of the effects of sedation.

We recommend that you limit strenuous physical activities including specifically those that might cause you to bend down or lift as these movements can strain the sutures.  Please do not elevate your blood pressure or get sweaty while healing.

Lastly, stay on top of follow-up appointments with Dr. Ben Paul to monitor the progress of your MOH’s reconstruction surgery recovery.

Combine your MOH’s Reconstruction Surgery

While the nature of your trauma will determine if you are viable for multiple procedures at the same time, patients may still consider additional surgeries once they have fully healed and with Dr. Paul’s blessing. If you have a specific area of interest that you would like Dr. Ben Paul to examine for a combined surgery, please let him know during your MOH’s Reconstruction surgery consultation as he will be able to provide you with his expert recommendations.
