What is an Otoplasty?

An otoplasty corrects ears or earlobes that are deformed from birth or injury, are too large or asymmetrical, are cosmetically damaged from long-term earring wear, or protrude too far out from the head. Dr. Paul understands that patients desire to wear their hair back or short. For this reason, Dr. Paul focuses on sculpting ears and earlobes that radiate natural beauty without distracting from the overall facial appearance.

Symmetry, shape, and proportion are the most important considerations when correcting the external ear. Since every face is different, Dr. Paul tailors ear repair to meet the concerns and facial structure of each patient.

What repairs can be done to the earlobes?

When considering earlobes, Dr. Paul is an expert in repairing:

Otoplasty Timeline


    When you meet with Dr. Paul, he will examine your ears and determine the best method of treatment. Once the two of you have agreed on the procedure, he will create your personalized surgical plan. Our Patient Coordinator will then help you select your surgical date and provide you with directions on how to optimize your surgical results.

    Otoplasty is performed under general anestheisa, whereas an ear lobe repair is generally performed under local anesthesia. For patients under general anesthesia, a medical clearance will be arranged in advance for safety.


    Dr. Paul will make discreet incisions in the creases behind your ears to perform the surgery. Once any cartilage or excess skin has been removed, Dr. Paul will suture back your ears into a natural resting position and you’ll be put into an otoplasty recovery headband to protect your ears and reduce swelling. Once released by our staff, you will be able to return home to recover in peace.

    Earlobe repair – Dr. Paul will mark the ear lobes for repair. The procedure is often performed under local anesthesia. The earlobes are cleansed and injected with local anesthesia. Repair then takes 30-60 minutes. Closure is with sutures. Earlobe repair is very well tolerated by most.


    There will be some soreness and pain, and you will need to continue wearing your recovery headband for otoplasty. Swelling will begin going down about a day after surgery, yet there will be some light swelling that will persist throughout the following months.


    Most return to the office in 7-14 days for suture removal and evaluation. You should be able to go back to school or work as well. You’ll still have some swelling, though you should see day-by-day progress. Most bruising has resolved by 2 weeks.


    You should be able to begin seeing the final results of your operation. You can expect to have a follow-up visit with Dr. Paul so that he can ensure you’re happy with your results and have been healing healthily. You will be able to enjoy your new ear contour for the rest of your life.

What to Expect on the Day of Otoplasty Surgery

On the day of your Otoplasty procedure, you will arrive at the surgical clinic and meet with Dr. Paul to review the procedure and ensure all your questions are answered. We will then mark the incision sites and explain the anesthesia process. After the anesthesia has taken effect, our surgical team will make incisions in the natural folds behind the ears. Depending on the type of Otoplasty being performed, cartilage may be removed, reshaped, or repositioned.

Ear lobe repair:

  • Earlobe repair does not involve any cartilage reshaping and is solely a correction of the soft tissue of the ear lobe.
  • Repair is often done awake with local anesthesia, though may be performed in combination with other procedures in the operating room.
  • Patients are given local anesthesia and sterile prep
  • Sutures are placed that are removed in 10 days
  • Re-piercing may occur 6 weeks after the date of closure


  • Otoplasty to change prominent ears is often performed in the operating room
  • While the phrase ‘ear pinning’ is commonly used, the reality of the procedure is quite different and involves no pins. Dr. Paul performs an otoplasty procedure by sculpting ear cartilage into a more appealing shape with sutures, which creates natural-looking results that do not showcase any signs of repair or modification.
  • If you are interested in external ear surgery, Dr. Paul will thoroughly explain the best options for the correction of your specific concern.

The procedure typically takes 1-2 hours to complete, after which the incisions are closed with sutures, and gauze is applied to protect the area.

Otoplasty Recovery


After Otoplasty, we advise our patients to protect their ears with a soft headband. After the procedure is complete, you will be monitored by our medical team for a short time to ensure that you are recovering well from the anesthesia. Once they are confident that you are stable, you can return home with a detailed list of post-operative care instructions to follow. It is essential to arrange for someone to drive you home and stay with you for 24 hours after surgery.

You should expect mild discomfort, which can be managed with prescription or over-the-counter medications. In general, patients require 7-10 days for complete recovery from their New York City Otoplasty.
