The Basics

What is a Rhinoplasty?

Commonly referred to as a “nose job”, this cosmetic surgery can include a variety of aesthetic and reconstructive techniques to improve breathing and the appearance of the nose, leading to a refreshed facial first impression. The procedure can be performed as a closed rhinoplasty or an open rhinoplasty.

Patients who seek more minor changes in their nose, like reducing a small bump, can be treated with a closed technique (i.e. incisions limited to the inside of the nostrils), but when patients need need tip work or more substantive changes, open rhinoplasty is used. The benefit to open rhinoplasty is a direct view of the nasal cartilage and bone allowing for the most accurate result with excellent lasting support. The approach that is ideal for you will be discussed during your consultation.

Using either technique during your nose reshaping, the skin is elevated and the nasal bones and cartilage are altered.

Rhinoplasty Timeline


    When you come in for your consultation, Dr. Paul will discuss your goals and examine your nose. We will take photographs and can visualize a projected change during the visit.  This will help you understand the aesthetic goal. After we determine we are an aesthetic fit, we will discuss the process of rhinoplasty including the pre-operative care, the surgical day and approach, and the after-care.  After your surgical plan is made, our Patient Coordinator will help you schedule your procedure date.

    1st Visit

    You are given the opportunity to meet with us again before your procedure day. At this visit, we will review the surgical plan as well as an instruction packet to guide you through your surgical experience. All medicines will be sent in before your procedure. We will guide you if you need a medical clearance.

    2nd Visit

    On the morning of surgery, you will see Dr. Paul and your anesthesiologist before your procedure.  After you’re administered anesthesia, Dr. Paul will clean your nose and perform your surgery.  Once he is satisfied with the results, you will be stitched up and a thermoplastic cast will be placed on the bridge of the nose. You should be able to go home and rest that same day.  You must have someone take you home.

    3rd Visit

    In the first few days, you will be sleepy and resting. Most patients take medicine for pain for the first few days. Patients have a feeling of heaviness in the nose and may have a stuffy nose. You may notice bruising around your eyes. Swelling peaks 3 days after surgery. You should limit activity to gentle walking and should not plan to work or go to school in the early recovery.

    (DAYS 1-7)

    You will return to the office to have the nasal cast removed. Please eat a meal before coming in for the visit. The cast removal day can be very emotional, and we look forward to supporting you with the unveiling of your nose. At this visit, we will review a list of instructions to help you return to normal activity.

    (Day 6 or 7)
  • Weeks Following Surgery

    Patient’s can return to work or school after their cast is removed.

    The biggest early risk in the first two weeks is a nose bleed, and so all exercise or physical exertion at this time should be avoided

    (WEEKS 1-2)

    Most of the facial bruising and swelling should have subsided by now. You will be able to resume very light exercise. You can expect another check-up appointment during this period. We will trim any sutures that have not dissolved.

    (WEEKS 2-4)

    Most patients are safe to resume full exercise and wear glasses 6 weeks after the procedure.

    (MONTHS 1-2)

    During this time, you will notice that two phenomena are occurring simultaneously. “Swelling” or trapped fluid is continuing to leave the nose and the “skin and soft tissue envelope is contracting”.  This means the skin is shrink-wrapping around the new cartilage and bone structure. Often in rhinoplasty, the nose is made smaller, and the skin will take time to contract over the year and show more definition.

    (MONTHS 2-12)

    At this point, we hope you are enjoying your rhinoplasty result. Dr. Paul may have you return so he can evaluate your recovery and possibly take some “after” photos for comparison.

    (12 MONTHS +)

Nasal Anatomy

From the earliest age, Dr. Paul has been fascinated by the nose. The complex balance between shape, size and function and the influence the nose has on overall facial harmony has continued to fuel Dr. Paul’s desire to help his patients get the best result possible. For all of its intricacies and nuances, the nose is incredibly compelling and through his journey to become a plastic surgeon, Dr. Paul has developed a thorough understanding of nasal anatomy.

When performing a Rhinoplasty, he consistently takes into consideration the importance of its structures, while seeking to preserve its natural function, but also improve the external appearance.

For your knowledge, we have included the portions of the nasal structure that Dr. Paul takes into consideration during this cosmetic plastic surgery here:

  • Skin – The main protector of your nose’s inner workings, the nasal skin in the upper portion of your nose is thinner and more mobile, while the lower portion is less flexible and thicker.  Different patients will have different skin thicknesses. The thickness of the skin cannot be changed in surgery. Patients with thicker skin will see less definition of the cartilage and bone and may have a longer period of swelling after surgery. Patients with thinner skin will see more definition. Dr. Paul will help you understand your skin thickness during the consult.
  • Bones – Underneath the skin, sits two arch-shaped bones at the top of the nose, which will vary in size depending on the individual. The bones project outward and are partially responsible for the hump that develops along the dorsum, often referred to as the bridge of the nose.
  • Cartilage – The nose has several distinct cartilage structures that help support and give your nose its primary shape. These different types of connective tissue in your nasal cavity are called the septal nasal cartilage, lower lateral cartilage, and upper lateral cartilage.

The septal nasal cartilage separates one nasal cavity from the other. The upper lateral nasal cartilage is a wing-like expansion, providing support within the nasal cavities. The upper lateral cartilage spans the middle third of the nose. The paired lower lateral cartilages make up the nasal tip. The cartilages are M-shaped and hold up each naris (nostril) and provide form and strength of your nostrils.

Types of Rhinoplasty

Whilst a general nose surgery is called Rhinoplasty, nasal surgeries can be divided into two groups: functional rhinoplasty (correcting breathing problems), and cosmetic rhinoplasty (correcting aesthetic problems).

Ethnic Rhinoplasty

The nose job has been available for decades. But for people of color in the U.S., getting a satisfying outcome often proves challenging because best practices among plastic surgeons were developed around the general facial shapes and aesthetic standards of Caucasians, who make up the largest patient demographic. However, a narrow, one-size-fits-all focus on the “American nose” fails to preserve patients’ ethnic identity, and it doesn’t take into account the rich diversity of facial shapes and characteristics in today’s society.

Applying Caucasian rhinoplasty techniques and guidelines to the face of an African-American, Asian, Native American, or Middle Eastern patient may result in dissatisfaction.

Revision Rhinoplasty

Sometimes after a Primary Rhinoplasty, the procedure doesn’t produce the desired results. The Revision Rhinoplasty is a reconstructive surgery designed to repair issues of the nose that are left unresolved after a previous surgery. Patients must wait a full 12 months after their first surgery before undergoing a Revision Rhinoplasty. Residual swelling can take up to a year to resolve, and for scar tissue to soften, making it difficult to accurately assess the nasal structures.

Septoplasty is a surgical procedure that changes the central wall between the right and left nasal passages. The septum is made of cartilage in the front and bone in the back. Dr. Paul is a dual board-certified ENT and Facial Plastic Surgeon. He can improve the septum independently but often will perform septoplasty during general rhinoplasty procedures.

The idea is to do the septoplasty at the same time as the rhinoplasty because the septal cartilage that is deviated or blocking the airway can be repurposed for nasal grafting. If the septoplasty is done before the rhinoplasty, valuable septal cartilage may be absent and alternative sources of cartilage may be used such as irradiated cadaveric rib cartilage, autologous rib cartilage, or ear cartilage.

What’s The First Step?

What to Expect on the Day of Rhinoplasty Surgery

In the morning of your procedure, you will meet with Dr. Paul and your anesthesiologist. We willr review the plan once more. During your Rhinoplasty performed, an IV will be placed and you will be administered full anesthesia to ensure your comfort.

Dr. Paul will perform the Rhinoplasty procedure using the appropriate techniques based on your unique nasal contour and desired outcomes. This may involve making incisions on the columella (skin between the nostrils), at the alar base (where the nostril meets the cheek) and/or inside the nostrils to access the nasal structures and reshape the bone, cartilage, and tissue of your nose.

A full rhinoplasty often takes around three hours, depending on the extent of adjustments made to your nasal elements.

Rhinoplasty Recovery

Surgical Rhinoplasty is an outpatient procedure, which means you can rest and recover comfortably in your home on the same day as your nose surgery. After surgery, you will be monitored in our recovery room until you are safe to be discharged. You will need a family member or friend available to drive you home as you will have had anesthesia. We will provide you with instructions on how to reduce swelling and achieve optimal results post-op.

You can expect some bruising and swelling around the eyes and cheeks, along with some mild discomfort and stuffiness the first week. As you recover, it’s important to keep your head elevated to achieve the best results. Due to the swelling, patients may temporarily notice blocked nasal passages and congestion, which will diminish on their own as healing continues.

Patients normally feel well enough to return to work in five days. You can gradually return to normal activities over the next 3 weeks, and return to more strenuous activities after six weeks. During this time, you will notice improved nasal and facial contours as the postoperative swelling continues to fade. And with time, the breathing will begin to improve.

It will take about a year for your new nose to settle into its final contour as final residual swelling resolves and the skin envelope continues to contract.
